STATUS
(ACT_STAT)

[ FIELD LENGTH 1 ] Refers to the status of the record in MCMIS. An Active status means the entity is currently in business and subject to the FMCSR, the HMR, or is an intrastate non-HM carrier issued a USDOT number by selected states. An Inactive status means the entity is no longer in business or is no longer subject to the FMCSR or the HMR. (A = Active, I = Inactive)

ENTITY TYPE
(CARSHP)

[ FIELD LENGTH 4 ] Refers to the type of operation in which the entity is engaged. It identifies the entity as a carrier, a hazardous materials shipper, a broker, a freight forwarder, a cargo tank facility, or a registrant. C = Carrier, S = Shipper, B = Broker, R = Registrant (entity who registers vehicles but is not a carrier), F = Freight Forwarder, T = Cargo Tank. Companies with multiple operations will have multiple letters.

USDOT NUMBER
(CENSUS_NUM)

[ FIELD LENGTH 8 ] This is the number assigned by MCMIS to a census record. It is sometimes referred to as the USDOT number. Each entity should have only one active census number. The census number has no internal coded structure. Numbers are issued sequentially as entities are added to the system.

NAME

The name used to identify the entity. The following data elements are included:

• LEGAL NAME. (NAME) [ FIELD LENGTH 120 ] This is the legal name of the entity.

• "DOING BUSINESS AS" NAME (NAME_DBA) [ FIELD LENGTH 120 ] This may be a trade name under which the entity does business. Any name identifying the entity other than the legal name.

FEDERAL TAX ID

FEDERAL TAX ID DATA will not be published in your File Extract. Due to privacy regulations, these fields are not disseminated to the general public.

• EIN NUMBER. (EIN_NUMBER) [ FIELD LENGTH 9 ] The Employee Identification Number used by the federal government to identify businesses/individuals.

• SSN NUMBER. (SSN_NUMBER) [ FIELD LENGTH 9 ] The Social Security Number used by the federal government to identify businesses/individuals.

DUN & BRADSTREET NUMBER
(DBNUM)

[ FIELD LENGTH 9 ] This is the corporate registration number given by Dun & Bradstreet.

PHYSICAL ADDRESS

Refers to the physical location of the entity's principal office. The following data elements are included:

• NATIONALITY. (PHY_NATN) [ FIELD LENGTH 2 ] General geographic location of the principal office. (US = United States and Territories, CD = Canadian Territories, MX = Mexican States, SA = Central and South American Nations)

• FMCSA REGION. (REG) [ FIELD LENGTH 2 ] FHWA/FMCSA Region in which the principal office resides. The following is a list of FMCSA Regions and the States assigned to each Region:
* Region 01: CT, ME, MA, NH, NY, NJ, RI, VT,   PR and VI
* Region 03: DE, DC, MD, PA, VA and WV
* Region 04: AL, GA, FL, MS, NC, SC, TN and   KY

• STREET. (PHY_STR) [ MAX FIELD LENGTH 50 ] Identifies the street address where the entity's principal office is physically located.

• CITY. (PHY_CITY) [ MAX FIELD LENGTH 25 ] Identifies the city where the entity's principal place of business is located.

• COUNTY CODE. (PHY_CNTY) [ FIELD LENGTH 3 ] Federally assigned number for a specific county within a state. {C}

• STATE. (PHY_ST) [ FIELD LENGTH 2 ] Identifies the state where the principal place of business is located. Two-letter postal abbreviation for the state. See Appendix A for state abbreviations.

• ZIP CODE. (PHY_ZIP) [ FIELD LENGTH 10 ] ZIP Code which corresponds to the street address of the principal place of business (any valid U.S. or Canadian ZIP code or CANADA, MEXICO or CENTRA). Example 37815-0310. If the expanded ZIP code is known, the field will contain the dash (-). Some Canadian and Mexican addresses contain the word CANADA or MEXICO as part of the ZIP code and Central American countries' ZIP codes contain CENTRA.

• UNDELIVERABLE PHYSICAL ADDRESS.[ FIELD LENGTH 1 ]

• TELEPHONE (AREA CODE, NUMBER). (TEL_NUM) [ FIELD LENGTH 13 ] The entity's telephone number at the principal place of business.

• CELL PHONE (AREA CODE, NUMBER).(CELL_NUM) [ FIELD LENGTH 13 ] The entity's cell phone number.

• FAX (AREA CODE, NUMBER). (FAX_NUM) [ FIELD LENGTH 13 ] The entity's fax number at the principal place of business.

NOTIFY FLAG
(NOTIFY_FLAG)

[ FIELD LENGTH 1 ] The Notify Flag is set to 'Y' when a company is newly added to MCMIS or is re-activated. The letter program picks up all census entries where this flag is 'Y' and processes them. It generates the appropriate type of notification letter and resets the flag to 'N'. (May be blank.)

(NOTIFY_DATE)

[ FIELD LENGTH 8 ] Contains the date the Notify Flag was set.

MAILING ADDRESS

Used when an entity receives mail at a location other than its principal place of business. The following data elements are included:

• NATIONALITY. (MAI_NATN) [ FIELD LENGTH 2 ] General geographic location of the entity's principal office. (US = United States and Territories, CD = Canadian Territories, MX = Mexican States, SA = Central and South American Nations)

• STREET. (MAI_STR) [ MAX FIELD LENGTH 50 ] Identifies the street used in the mailing address.

• CITY. (MAI_CITY) [ MAX FIELD LENGTH 25 ] Identifies the city used in the mailing address.

OFFICER IN CHARGE (OIC) CODE
(OIC)

[ FIELD LENGTH 2 ] The code representing the FMCSA Officer-in-Charge having jurisdiction over the entity.

SAFETY INVESTIGATOR TERRITORY CODE
(TERR)

[ FIELD LENGTH 2 ] Code assigned to the territory (State or County for which an OIC has responsibility) in which the entity's principal business is domiciled.

ICC DOCKET NUMBERS

[FIELD LENGTH 2, FIELD LENGTH 6] (ICCDOCKET1, ICC1), (ICCDOCKET2, ICC2), (ICCDOCKET3, ICC3), Federally-assigned Interstate Commerce Commission entity's identification number. Space allotted for three Docket Number Prefixes (usually MC, MX, or FF) and three ICC Docket Numbers. The terms MC number or MX number have replaced the term ICC number in general usage.

 

 

 

 

 

 

CLASSIFICATION
(CLASS)

[ MAX FIELD LENGTH 12 ] Identifies the type of entity. MCMIS recognizes the following classifications for entity type (NOTE: An entity can have more than one type of classification. A letter will appear in all those fields representing classifications that apply):

• A = Authorized For Hire. A commercial entity whose primary business activity is the transportation of property/passengers by motor vehicle for compensation.

• B = Exempt For Hire. A for-hire entity transporting commodities or conducting operations not subject to economic regulation by the Interstate Commerce Commission.

• C = Private (Property). An entity whose highway transportation activities are incidental to, and in furtherance of, its primary business activity.

• D = Private Passenger (Business). A private entity engaged in the interstate transportation of passengers which is provided in the furtherance of a commercial enterprise and is not available to the public at large.

• E = Private Passenger (Non-Business). A private entity involved in the interstate transportation of passengers that does not otherwise meet the definition of a private entity transporting passengers (e.g., business, church buses).

• F = Migrant. An entity who transports in interstate or foreign commerce at any one time, three or more migrant workers to or from their employment (refers to "contract carrier by motor vehicle").

• G = U.S. Mail. A entity which transports U.S. Mail.

• H = Federal Gov't. Transportation of property or passengers by a U.S. Federal Government agency.

• I = State Gov't. Transportation of property or passengers by a U.S. State Government agency.

• J = Local Gov't. Transportation of property or passengers by a local municipality.

• K = Indian Tribe. Transportation of property or passengers by an Indian tribal government.

• L = Other Classification. An entity which does not fall into one of the classifications above.

• Other Defined. (CLASSDEF) Column [ FIELD LENGTH 50 ] Identifies other classification.

CARRIER/SHIPPER OPERATION

[ MAX FIELD LENGTH 6 ] (CRRINTER*, CRRHMINTRA*, CRRINTRA*, SHPINTER*, SHPINTRA, VEHICLE_REGISTRANT*) Identifies the carrier as being engaged in interstate, intrastate hazardous material or intrastate non-hazardous material transport activities. In cases of shippers, it identifies the shipper as being engaged in interstate or intrastate shipping activities. Also allows for indication of Vehicle Registrants. A letter will appear in all those fields representing operations that apply.

BUSINESS ORGANIZATION
(ORG)

[ FIELD LENGTH 1 ] Identifies the type of legal structure used by the entity. 1 = Individual, 2 = Partnership, 3 = Corporation. From reviews only.

CARGO CLASSIFICATIONS

[ MAX FIELD LENGTH 30 ] Identifies the type of cargo transported by the entity. MCMIS recognizes the following cargo types. (NOTE: An entity can transport more than one type of cargo). The letter "X" will appear in all those fields representing cargo that apply:

• # DRIVERS/INTERSTATE/WITHIN 100 MILE RADIUS.(INTERLT100) [MAX FIELD LENGTH 5] Identifies the number of drivers hired by the entity to drive interstate within a 100 mile radius.

• # DRIVERS/INTERSTATE/BEYOND 100 MILE RADIUS. (INTERGT100) [MAX FIELD LENGTH 5] Identifies the number of drivers hired by the entity to drive interstate beyond a 100 mile radius.

• # DRIVERS/INTERSTATE/TOTAL. (INTER_DRS) [MAX FIELD LENGTH 6] Identifies the total number of drivers hired by the entity to drive interstate.

Contains data regarding a federally-conducted or state-conducted review of a carrier's/shipper's compliance with the Federal Motor Carrier Safety Regulations (FMCSR) and Hazardous Material Regulations (HMR). These reviews result in the carrier receiving a Federal safety rating of Satisfactory, Unsatisfactory or Conditional. • LATEST REVIEW TYPE. (REVTYPE) [FIELD LENGTH 1] This is the type of review conducted. (C = Compliance Review, H = Shipper Review, N = Non-Ratable Review, A = Safety Audit, S = Safety Review, E = Educational Contact Review, U = Historical Audit Record, B = Automatic Safety Assessment Program Review, F = CTFR Only, G = CR and CTFR, I = Carrier Inactivate, J = CR and SECURITY CONTACT REVIEW, or Null.)

• LATEST REVIEW DOCUMENT NUMBER. (REVDOCNUM) [ FIELD LENGTH 8 ] Number assigned by MCMIS to a review after it has been added to MCMIS.

• LATEST REVIEW DATE. (REVDATE) [ FIELD LENGTH 8 ] The date the review was completed by a Safety Investigator.

• RECORDABLE ACCIDENT RATE. (ACC_RATE) [ FIELD LENGTH 6 ] The rate is computed from data taken from the latest Safety/Compliance/Educational/Review. Total Recordable Accidents multiplied by 1,000,000 miles divided by the mileage.

• PREVENTABLE RECORDABLE ACCIDENT RATE. (REPPREVRAT) [ FIELD LENGTH 6 ] The rate is computed from data taken from the latest Safety/Compliance/Educational Contact Review. Total Preventable/Recordable Accidents multiplied by 1,000,000 miles divided by the mileage.

* A record that contains 000.000 as the accident rate has two possible meanings:
 (1) No mileage data is available to compute the accident rate or
 (2) mileage data exist and the actual accident rate is 000.000.
* If the record contains 999.999 this indicates that the accident rate is greater than 10.0.

MCS-150 MILEAGE CALENDAR YEAR
(MLG150)

[ FIELD LENGTH 10 ] The number of miles reported by the entity on the form MCS-150.

REVIEW MILEAGE
(MILETOT)

[ FIELD LENGTH 10 ] The number of miles reported on the motor carrier/shipper from the most current Review.

SAFETY RATING
(RATING)

[ FIELD LENGTH 1 ] This is a measure of the carrier's compliance with the Federal Motor Carrier Safety Regulations (FMCSR).

• Motor carriers rated before 1986 were assigned safety ratings based on previous procedures used to determine the carrier's compliance with the FMCSR such as a Safety Management Audit, or a Re-investigation conducted on the carrier's operation,

This covers all major areas of the Safety Regulations. Much of the rating is based on an interview with management. The main purpose is to educate a carrier about the FMCSR. A Compliance Review is a statistical sampling or audit of required records to determine a motor carrier's compliance with the FMCSR. An in-depth review of the motor carrier's records is made to check different categories of drivers, vehicles and trips. This review may be limited to specific problems and may include a detailed investigation of a problem area. The compliance status determination is based on FMCSR violations discovered during the review.
